The ViTelCare™ T400 Home Health Monitor allows cost-effective, in-home management of chronically ill patients post-discharge.  It supports daily patient health assessment, disease education and vital sign capture.
By alerting care providers to potential health problems before they escalate, the ViTelCare T400 helps reduce the number of in-home visits needed and prevents hospital readmission.  A broad range of chronic primary disease programs and common co-morbid combinations are available to easily tailor the health assessment to each patient.

Essentially a mini computer, the ViTelCare T400 monitor was developed with leading-edge technology and designed with input from patients and care providers alike. The compact, portable device features an 8" touchscreen display that makes it easy for patients to view and enter information.
