The ViTelCare T400 Home Health Monitor supports the safety and well-being of patients with congestive heart failure, diabetes, chronic obstructive pulmonary disorder (COPD), asthma, or mental health issues, as well as many combinations of these conditions.
By providing a daily assessment of the patient’s vital signs and symptoms, the monitors alert you to changing conditions, so you can intervene before a home visit or hospitalization is required.
The VitelCare Web brings patient monitoring capabilities to the case manager’s computer desktop using Windows® Internet Explorer. Using VitelCare Web, healthcare professionals can monitor incoming patient data.
How ViTelCare Works
- The portable, lightweight ViTelCare monitors are set up at the patient’s home. The patient enters and views information via the large touchscreen display.
- Vital signs devices, such as a pulse oximeter or blood pressure monitor, can be connected to the ViTelCare device, or patients can take readings manually and enter the measurements.
- The ViTelCare monitor engages the patient in a daily questionnaire related to specific disease condition(s). Disease education is incorporated in the interaction to promote better self-care.
- After each session, the data is sent over standard telephone line, LAN/WAN or broadband platforms to the care provider.
